This requirement is for people who are
1. PRC License and IBP License holder
2. Residing or Working in Quezon City
If you are a newly license PRC holder, you need to bring the following,
1. Present PRC ID or IBP ID
2. Php 300
For those who will renew their own PTR, below are the following requirements
1. Previus Official Receipt
2. Present PRC ID Or IBP ID
3. 300 Pesos
While for those who will get the PTR via a representative, below are the following requirements,
1. Previous Official Receipt
2. Photocopy of PRC ID or IBP ID
3. Authorization Letter
4. Photocopy (Valid ID of Representative)
For those who will transfer from other Cities to Quezon City
1. Previous Official Receipt
2. Present PRC ID or IBP ID
If you don't have your original PTR but have a xerox copy of it just bring it and prepare an additional 50.00 pesos for the verification fee.
Procedure
1. Go to the annex building ground floor. The entrance is beside mayaman street.
2. There is a seperate line for getting a PTR so you should ask the guard that you will get a PTR
(As of this time Window 12 to 14).
4. Pay the corresponding fee in the same window.
5. Get your new PTR. Check if the information is correct.
It takes about 5-10 mins. to get a PTR.

No forms to be filled up. For Quezon City you can get the PTR in the Quezon City Hall. For other cities I think you can get it in the city Hall

ReplyDeleteA fee of Php300 will be charged every month of January (renewal month) whether application is new or renewal. A penalty of Php75 plus 2% (Php82.50) will be added if payment will be made on the month of February. x2% for the succeeding months (March-December).
DeleteHi! Magtatanong lang po about sa PTR. I'm from San Jose Del Monte Bulacan kaya don po ako kumuha ng PTR noong new registered po ako kaso sa QC na po ako nagwowork. May nabasa ako na dapat kung san ang workplace dun din kukuha. Paano po un kukuha po sana ako for this year ng 2020. Thank you in advance po sa sasagot :)

Why not acknowledging to get PTR? The license is not yet revocated. Yes, the license is expired kaya nga po kumukuha ng PTR because this is one of our requirements to renew our license. If you will not authorizing to get our PTR, how can we renew our license
ReplyDeleteHi last time po ako kumuha ng ptr way back 2016 po. di ko na sya na renew and di ko na nagamit. ngayon po.balak ko kumuha ulit. years po ba penalty nito or for this year lang. and hindi ko na makita luma kong ptr.
